Waiver

A waiver will be required to take our class.  If you would like to download and complete it ahead of time you can do so by clicking the waiver and downloading.  Then you can save it and email it or print and bring with you.

Printed forms will be available at all classes 

Please arrive to the class at least 10 minutes early to allow time to fill out the waiver and get prepared for the class to start.
